This program provides support to youth who are transitioning from group home care. The program is designed for youth under 18 years of age who are in care with the Ministry of Children and Family Development.
Family care homes provide support, mentoring and skill enhancement for one youth placed in their home. The program offers help to youth to further develop the life management skills to successfully transition to adulthood. Each youth will have the opportunity to live in a home which will provide a separate living space to start experiencing greater independence while receiving the ongoing support and interactions with the family in the home.
A foster parent is at home and available to the youth at all times during their stay. Foster parents receive support through mentoring and relief from the program faciliator and work collaboratively with the youth. The program facilitator is available to the youth to provide support for life management and day activities. Through this program foster parents have access to the program facilitator 24 hours per day for emergency support, consultation, crisis intervention, debriefing, mediation and training.
